Rehoming Agreement

When an interested party contacts you about your pet, the first step is to conduct a thorough telephone interview.  Ask many questions, including the following:
Why are you interested in my dog?

Where will the dog live during the day?

Where will she sleep at night?

What kind of activities do you want to do with the dog?

Do you plan giving her additional training?

Where will you take the dog for veterinary care?

Do you have any other pets?

Do you have a family or roommates living with you?

Have you ever had a dog/cat before? If so, what happened to him or her?

What would you do if you had to give up the pet for some reason in the future?

Let the prospective adopter know if you’re willing to take the pet back or not in the event that things don’t work out.  It is always a good idea to get your pet up to date on shots and spay/neutered before rehoming. 

SWAT will ask for a rehoming fee for your pet when it’s posted.  When you sign the agreement, you are agreeing to have the adopter complete either an on-line or paper SWAT Adoption Application. SWAT should receive that application, along with the  pet’s adoption fee within 1 week of the pet's adoption to cover our costs of rehoming your pet and allowing us the information to follow up to help ensure your pet is in a safe, loving home.  If you fail to have the adopter complete an on-line or paper application and/or fail to collect the adoption fee, you will be responsible for completing the app with the adopter's information and paying the fee yourself immediately upon the pet's adoption.  There will be no fee collected and no reimbursement if the pet goes on transport to an out of state rescue facility, however, you may be required to pay a $25. Health Certificate fee.

It is best to take your pet to the new adopter’s home instead of letting them take your pet from your home; that way, you can see where your pet will be living.  You will want to ask about shelter/shade when the pet is outside.  You will also be able to see how the family interacts with your pet. 
Moving to a brand new home will be very stressful for a pet.  Explain to the new family that it will take a few weeks for the pet to get used to the new family, new rules, etc. and that they should be patient.  If there is any concern, leave your phone number so that they can reach you.  You can also give them our card so they can call us for advice on behavioral issues.
Remember, helping your pet find its new home is ultimately your responsibility. You are front lines to meet the adopter and make the decision of whether you think it is a good fit for your pet.  If you are uncomfortable with the adopter, it is best to pass and continue to look for a new home.  If you have any questions during the process of would like to check on your pet during the first 6 months, please feel free to call us.

PET REHOMING AGREEMENT
SWAT requires verification that your pet is neutered and up to date on their rabies shot.  We also require full disclosure regarding behavioral issues with your pet.  You MUST tell us if this animal has any aggressive tendencies or has every bitten anyone or attacked another animal.  Full disclosure will ensure your pet goes to a family that is aware of any difficulties and willing to work with them. 
I understand that SWAT is agreeing to post my pet on the internet adoption and social media sites, answer calls, emails, etc. to help my pet get adopted.  SWAT does not guarantee placement and expects the pet’s owner to take an active role in helping to rehome the pet. If we are unable to make contact with you after 14 days, we will assume you no longer need help with rehoming your pet and all information will be removed from SWAT on-line platforms.
I agree to notify Saving Whiskers and Tails when my pet has been rehomed.  

I understand that I can contact Saving Whiskers and Tails for an update on my pet during the first 6 months.
I understand that if my pet’s new home does not work out for any reason, that the pet remains my responsibility.  Saving Whiskers and Tails will continue to work with you should it come back to you after rehoming.  A new rehoming fee will be required.  
I understand the above information and agree to the rules.
